A 65-year-old male presents to the primary care clinic with central chest pain lasting 2 hours. The pain is dull, radiates to the left arm, and is associated with mild shortness of breath. It is worse on exertion and improves slightly with rest. He denies nausea, vomiting, or fever.

Pulse: 88 bpm, regular.

Blood Pressure: 132/78 mmHg.

Respiratory Rate: 16/min.

Oxygen Saturation: 97% on room air.

Temperature: 36.8°C.

Hypertension: Managed with Ramipril.

Ex-smoker: Quit 10 years ago.

Alcohol Use: None.

Ramipril: 5mg once daily.
